Special Services
Chronic Care Management (CCM)
If you have two or more chronic health conditions, this program is designed to support you. You'll be paired with a dedicated nurse as your main point of contact. Your nurse will answer questions, provide education, and share helpful resources. This service is non-face-to-face—you can connect with your nurse by phone or secure messaging whenever it's convenient for you. We'll also keep your entire care team updated so everyone is on the same page.
Remote Patient Monitoring (RPM)
If you have uncontrolled high blood pressure or recently changed blood pressure medications, this program can help. We'll provide you with a blood pressure monitor that securely sends your readings from home to our team. Your assigned nurse will track your results, offer education on lifestyle changes, and notify your provider if adjustments are needed.
Principal Care Management (PCM)
If you're managing a single chronic condition (like lupus or rheumatoid arthritis), PCM gives you focused support. You'll be paired with a nurse who understands your condition. Your nurse can help with medications, scheduling, questions, and any new health concerns. Think of this program as an extension of your specialist, giving you personalized care between appointments. We'll make sure communication is clear and your entire care team stays informed. Connect by phone, text, or both—whatever works best for you.
Remote Therapeutic Monitoring (RTM)
For patients with rheumatology conditions, RTM combines personalized support with easy-to-use technology. Through our app, you can track medication use, treatment progress, and report any pain or concerns. Your nurse will review your updates, monitor your symptoms, and provide support between visits.
Transitions of Care Management (TCM)
TCM helps you safely transition home after a hospital stay. Within a few days of discharge, a Registered Nurse will call to review your medications and answer urgent questions. They'll also schedule a follow-up visit with your primary care provider (usually within 1–2 weeks).
